Jodeme’s passion is convening opportunities, building capabilities, and inspiring those in health and social systems to work together to realize the potential of the collective to achieve population health and wellbeing. She serves as a catalyst to support large-scale human transformative change and ensures change opportunities are co-designed.
Jodeme is a recognized leader in systems-level transformation in health and social care. In addition to founding 4C Impact, she is the co-founder of International Foundation for Integrated Care (IFIC) Canada, the North American Centre for Integrated Care at the University of Toronto's Dalla Lana School of Public Health and serves as Vice-Chair on the Board of IFIC. She also Co-leads the CIHR-funded Network for Integrated Care Excellence (NICE), advancing the Transforming Health through Integrated Care initiative.
With over 25 years of experience, Jodeme is a leadership coach, Strategic Advisor, Capability Builder, Convenor, and Breakthrough Facilitator dedicated to achieving population health and wellbeing. She is a Fellow of Dr. Helen Bevan, an Adjunct Faculty member at the University of Toronto’s Institute for Health Policy, Management and Evaluation, and a member of the leadership team of the Health Leadership Academy at McMaster University.
Jodeme’s contributions to integrated care and transformative solutions have earned her numerous accolades, The top 10 Women Changing Health Care in Canada, the National Catalyst for Change Award, Women of Inspiration, two National High Impact Practice Awards, Canadian Home Care Association and two Ontario Ministers of Health Medals. She also Co-leads the CIHR-funded Network for Integrated Care Excellence (NICE), advancing the Transforming Health through Integrated Care initiative.
