The collective energy in the room at the inaugural Integrated Care Action Summit was a powerful reminder of what becomes possible when we lead together, united around advancing integrated care and population health! On November 24th and 25th, all 58 OHTs self-convened alongside Primary Care Networks (PCNs), sector partners, lived-experience leaders, Ontario Health partners and Ontario’s Ministry of Health team for the first time since their inception.
This was a Summit built by the people who lead in every local community across our province, and Jodeme was invited as one of only two keynote speakers, alongside Dr. Jane Philpott, Chair of Ontario’s new Primary Care Action Team.
Jodeme also led a mini masterclass on collective leadership.
Building on Dr. Philpott’s opening keynote, she emphasized that primary care is not just another priority — it is the strategic lever for system transformation. High-performing primary care shifts the centre of gravity toward prevention, continuity, coordinated care, and population well-being. But primary care cannot carry the full complexity of people’s lives on its own. It must be integrated with the rest of health and social care to create an ecosystem that is resilient, equitable, and capable of delivering population-level impact. Integration is what turns strong primary care into a high-performing system.
